ATHLETE MEDICAL FORM INFORMATION
The Waynesburg Athletic Training Staff is committed to providing our student athletes with the highest quality medical care possible. In order to do this, we ask that you provide our athletic trainers and team physicians with some background medical and insurance information.
There are four components to completing your athletic training paperwork:
1. Creation of an ATS profile
2. Completion of paperwork, found in “Forms” tab on ATS
3. Provision of sickle cell test results (can be uploaded to ATS profile or faxed/emailed to AT staff)
4. Completion of Impact Test (instructions found below)
1. ATS PROFILE:
Waynesburg University Athletic Training uses a computer based record-keeping system (ATS). Creation of an account is mandatory for participation in athletics at Waynesburg University.

ALL FIRST TIME USERS MUST ENTER THE FOLLOWING LOGIN CREDENTIALS TO ACCESS THE SYSTEM:

2. ATS FORMS:
After creating your ATS profile and finishing the General, Sickle Cell, Insurance, and Contact tabs and clicking “Save”, a new tab will populate at the top of the page: Forms. You will note a drop down box titled “Form Name”, with nine form options (Insurance Policies, Informed Consent, Liability, HIPAA, Concussion Statement, Impact, Medical History, ADHD, and Sickle Cell). Complete each of these forms by selecting from the drop down list, hitting “New”, completing each section, and clicking Save. Your forms will then be listed below when completed along with the date of completion. Please ensure they are fully completed.

3. SICKLE CELL TEST:
Beginning in fall 2022 the NCAA began requiring a sickle cell test result be provided for all division III student-athletes. This is a mandatory part of participating in athletics at Waynesburg University. These results can often be found in newborn screening paperwork, otherwise a new blood test will need to be requested. Attached below is a letter that can be provided to your physician describing why the test is required for NCAA athletes. Click

4. IMPACT TEST:
The impact test is a concussion test that the AT staff utilizes as a tool for return to play decisions following a concussion. The baseline of this test must be completed prior to your arrival on campus. This test only needs to be completed once in your time at Waynesburg, unless team physicians decide otherwise. Here is the
